MEDICAL DISCLAIMER & EMERGENCY NOTICE OF C&R MEDICAL CARRIER

EMERGENCY SERVICES NOTICE
FOR MEDICAL EMERGENCIES, ALWAYS CALL 911
C&R Medical Carrier provides non-emergency medical transportation only. Our services are not appropriate for medical emergencies requiring immediate care, continuous monitoring, or advanced life support.
Call 911 immediately if you experience:
- Chest pain or heart attack symptoms
- Difficulty breathing or shortness of breath
- Stroke symptoms (facial drooping, arm weakness, speech difficulty)
- Severe bleeding or trauma
- Loss of consciousness or altered mental status
- Seizures
- Suspected overdose or poisoning
- Severe allergic reactions
- Any life-threatening condition
Do not delay emergency care by waiting for non-emergency transportation.
SERVICE LIMITATIONS
What We Provide: C&R Medical Carrier provides transportation services for stable patients who do not require emergency medical care during transport. Our drivers are trained in CPR and first aid but are not paramedics, EMTs, or licensed healthcare providers.
What We Do NOT Provide:
- Emergency ambulance services
- Medical diagnosis, advice, or treatment
- Advanced life support or continuous medical monitoring
- Nursing care or personal care services
- Medical decision-making or healthcare recommendations
- Transportation for unstable patients requiring emergency care
NO MEDICAL ADVICE
Not a Substitute for Professional Medical Care: Information provided by C&R Medical Carrier staff, including drivers and dispatchers, is for transportation coordination purposes only and does not constitute medical advice. Never rely on transportation staff for medical guidance or decisions.
Consult Healthcare Professionals: Always consult with your physician, nurse, or other qualified healthcare provider for:
- Medical advice, diagnosis, or treatment
- Questions about your health conditions
- Decisions about medications or treatments
- Assessment of symptoms or medical concerns
- Discharge instructions or post-procedure care
Driver Observations: While our drivers may observe and report concerning symptoms during transport, these observations are not medical assessments. Drivers will contact emergency services if they observe life-threatening conditions, but they do not provide medical evaluation or treatment.
MEDICATION DELIVERY DISCLAIMER
Pharmaceutical Responsibility: C&R Medical Carrier transports medications as prescribed by your healthcare provider and dispensed by licensed pharmacies. We do not:
- Provide advice about medication use, dosage, or administration
- Make determinations about medication appropriateness
- Verify medication accuracy (this is the pharmacy’s responsibility)
- Provide pharmaceutical counseling
- Make changes to prescriptions or medications
Questions About Medications: Direct all questions about your medications to your prescribing physician or pharmacist. We transport medications safely but do not provide pharmaceutical expertise.
Medication Safety: While we follow FDA and state regulations for temperature-controlled transport, we are not responsible for medication effectiveness issues unrelated to transportation. Contact your pharmacist immediately if you have concerns about medication quality or safety.
PATIENT RESPONSIBILITY
Medical Clearance: Clients using our transportation services must be medically stable and cleared for non-emergency transport. Healthcare facilities are responsible for determining medical stability before discharge or transfer.
Accurate Information: You are responsible for providing accurate information about your medical conditions, mobility limitations, and special needs to ensure safe transportation. Failure to disclose relevant medical information may result in unsafe conditions.
Following Medical Advice: We transport you to and from medical appointments, but we are not responsible for ensuring you follow medical advice, attend appointments, or comply with treatment plans. Healthcare decisions remain between you and your healthcare providers.
ASSUMPTION OF RISK
By using our services, you acknowledge and accept that:
- Medical conditions can change unexpectedly during transport
- Transportation involves inherent risks, including vehicle accidents
- We provide non-emergency transportation for stable patients only
- You assume responsibility for determining whether non-emergency transport is appropriate for your situation
- In case of a medical emergency during transport, our drivers will contact 911 and follow emergency protocols
RELEASE OF LIABILITY FOR MEDICAL OUTCOMES
C&R Medical Carrier is not responsible for:
- Medical complications unrelated to transportation services
- Progression of existing medical conditions
- Adverse reactions to medications we transport
- Medical outcomes resulting from missed appointments (unless caused by our negligence)
- Healthcare provider decisions or the quality of medical care received
- Medication effectiveness or pharmaceutical issues beyond our control
WHEN IN DOUBT, SEEK EMERGENCY CARE
If you are uncertain whether your condition requires emergency care or non-emergency transportation, err on the side of caution and call 911 or go to the nearest emergency department.
Do not use our services if:
- You are experiencing a medical emergency
- Your condition is unstable or deteriorating
- You require continuous medical monitoring
- You need immediate medical intervention
- A healthcare provider has advised emergency transport

By scheduling or using C&R Medical Carrier services, you acknowledge that:
- You have read and understood this Medical Disclaimer
- You understand the difference between emergency and non-emergency medical transport
- You are medically stable and appropriate for non-emergency transport
- You will seek emergency services (911) for medical emergencies
- You will not rely on our staff for medical advice or decisions
- You understand the limitations of our services
